Our office staff are available Monday through Friday from 9:00 am to 5:00 pm. Please note that unfortunately, emergencies do occur and will occasionally cause delays on our schedule. We always try our best to honor your appointment time. If you cannot keep a scheduled appointment, please give us at least 24 hours’ notice so that another patient may use your time.

Our services include same day appointments, new patient appointments, and urgent surgical appointments. If you need to reach us after regular office hours, please call our main number: (978) 922-9226 and our answering service will assist you. If your call is urgent, they will have the provider on call contact you directly.

New patient appointments generally require more time than established patients, as we need to learn about your medical history and current concerns in order to provide you with the best possible care. Please print out the new patient forms below, fill them out and bring them with you to your first visit. Please arrive 15 minutes prior to your appointment. Your help and cooperation is greatly appreciated.

Upon arrival to your appointment, please have the following documents ready:

• Your driver’s license or photo ID
• Medical insurance card
• List of current medications/allergies
• Any pertinent medical records
• Copy of the referral (if necessary)
• New patient forms ( New patients only)

Co-payments are collected at the time of check-in for your visit. All major credit cards, cash and personal checks are accepted.

Essex Surgical Associates offers My Lahey Chart, an exciting way to communicate with your doctor! Our physicians are enrolled in My Lahey Chart providing access for patients to communicate with our office.

We believe My Lahey Chart will increase accessibility to your physician and all the services of the office, and at the same time, balance the practice’s commitment to provide you with the best customer service possible. However, if you prefer, you may always contact your physician or the office staff at (978) 922-9226, Monday through Friday during our business hours of 9 am-5 pm.

To send a confidential message to Essex Surgical Associates Staff:

• Visit MyLaheyChart.org to set up an account
• Select the “Mail” menu option
• Click on “Write”, your providers name will default in
• At the “About” prompt, select “My Care”, write your message
• Click on “Send” when you are finished

We accept most medical insurances. Since each insurance plan is different, we ask you to check with your particular plan administrator to obtain your guidelines before visiting our office. Please feel free to discuss fees with us and advise us of any unusual financial problems you may have.

Please note that Essex Surgical Associates is a privately owned practice. If you receive any services from Beverly Hospital such as lab tests, image scans and hospital based services, they will bill you directly. Please be sure to obtain a referral from your primary care physician’s office before your visit with Essex Surgical Associates, if required by your insurance policy.
